Forms for CMES / MA Graduate Students

You will need Adobe Reader or Apple Preview to access the forms. All forms must be emailed to Lisa Parisi as typed, digitally-signed PDFs. Scans will not be accepted.  


Advising & Registration Form

Each semester's coursework must be approved before registration. The student must communicate the changes to the graduate advisor and Lisa Parisi to ensure degree requirements are met. All courses must be taken for a letter grade.

Master’s Thesis Enrollment Form

Thesis arrangements (selection of the topic of study, supervising instructor, and a reader) must be finalized and approved by the graduate advisor before you can register. The supervising instructor must be a member of the MES Graduate Studies Committee (GSC)

Master's Report Enrollment Form

Report arrangements (selection of the topic of study, supervising instructor, and a reader) must be finalized and approved by the graduate advisor before you can register. The supervising instructor must be a member of the MES Graduate Studies Committee (GSC).

Internship Registration

For students who wish to earn graduate credit. Instructions and requirements are listed on the form.

Non-MES Course Credit Petition

This form is used to petition for a course in another department (not cross-listed with MES) to be counted toward the MES degree requirements. A minimum 30% Middle Eastern content is required, and your request must be approved by the graduate advisor by the twelfth class day.

This form is for you to keep track of your progress toward your degree. Please update it regularly and share it with the Academic Advisor and Graduate Coordinator during Advising sessions. 


Course Restrictions

Center students may not apply more than two total of the following courses for the MA degree:
  • MES 382 conference course (of the independent study type);
  • MES 382 conference course (of the bump-up type);
  • upper-division course (not bumped-up)

MES 382 Conference Course Agreement

Conference course arrangements must be finalized and approved by the graduate advisor before you can register. Only two conference courses may be applied to a master's degree, including 'Bump-Ups'. 

MES 382 Bump-Up Conference Course Agreement

Bump-up course arrangements must be finalized and approved by the graduate advisor before you can register. Only two MES 382 courses may be applied to a master's degree, including Conference Courses, 'Bump-Ups'

Undergraduate Course Enrollment Form

With graduate advisor permission, students are eligible to enroll in an upper-division course. The last two digits indicate the rank of the course; if they are 01 through 19, the course is of lower-division rank; if 20 through 79, it is of upper-division rank. Only a combination of two MES 382s (in either form) and/or upper-division courses may be applied to a master's degree.

Late Registration

For continuing students who did not register/pay/confirm attendance during pre-registration. Late registration is in effect starting 2 days before the 1st class day of each semester. It is the student’s responsibility to gather all the required signatures and deliver this form to the Graduate School, which is only needed after the 12th class day of a long semester (or 3rd class day in summer).

Leave of Absence

For master’s students seeking authorization for a single semester or academic year leave.
