Forms for CMES Graduate Students

Academic Forms

  • Graduate Degree Planner
    • View your coursework, plan your course schedules, and see your program of work.  To be used to fill out the Master's Checklist. Another place you can view your grades is through the Grade Report.
  • Master’s Thesis Enrollment Form
    • Thesis arrangements (selection of the topic of study, supervising instructor, and a reader) must be finalized and approved by the graduate advisor before you can register.
  • Master's Report Enrollment Form
    • Report arrangements (selection of the topic of study, supervising instructor, and a reader) must be finalized and approved by the graduate advisor before you can register.
  • Internship Registration
    • For students who wish to earn graduate credit. Instructions and requirements are listed on the form.
  • Non-MES Course Credit Petition
    • This form is used to petition for a course in another department (not cross-listed with MES) to be counted toward the MES degree requirements. A minimum 30% Middle Eastern content is required, and your request must be approved by the graduate advisor by the twelfth class day.
    • Students should use this form to apply for the Dual Language Track. Students should apply prior to their second year in the program.

  • Late Registration
    • For continuing students who did not register/pay/confirm attendance during pre-registration. Late registration is in effect starting 2 days before the 1st class day of each semester. It is the student’s responsibility to gather all the required signatures and deliver this form to the Graduate School, which is only needed after the 12th class day of a long semester (or 3rd class day in summer).
  • Leave of Absence
    • For master’s students seeking authorization for a single semester or academic year leave.
